It tells you what the provider billed Medicare, Medicare's approved amount, the amount Medicare paid, and what you have to pay. It is not a bill. ( See also Medicare Summary Notice (MSN).)
The statement of payment from Medicare; it shows the amount charged by the provider, the amount approved by Medicare and the amount actually paid by Medicare. It is the statement that is submitted to the insurance company for payment under the Medigap policy. Other insurers sometimes use the term explanation of benefits (EOB) to refer to their own payment statements.
